Our pharmacists and pharmacy technicians work together with you and your physicians, nurses, and other health care providers to ensure that your medication use is safe and effective.

We provide an integrated medication delivery system for all inpatient areas which is supported by an electronic medication administration record and barcode medication verification. We do this to ensure your medications are safely administered by nursing staff. One of our department’s priorities is to ensure that you and all our patients receive the “right drug” at the “right time” and the “right dose” – every time.

Medication Reconciliation

If you are admitted to Oak Valley Health, you will have a medication history completed by a pharmacist or pharmacy technician. Having an accurate and up-to-date list of home medications reduces the risk of medication errors during your stay. Remember to bring all your medications with you when you come into our hospitals.

Medication Management

We will monitor, recommend, or make changes to your drug therapy throughout your stay. If you have questions or concerns about any of your medications during your stay, ask to speak to a pharmacist.

Antibiotic Stewardship Pharmacy Team

Pharmacists are part of a multidisciplinary team to assess patients receiving antibiotics in our hospitals. Our goal is to use the right drug, right dose, and for the right duration to prevent complications related to the use of antibiotics. Education

The pharmacy team can provide ongoing education to you and our other patients, clinical staff, and physicians in a variety of settings and formats.
