If your family physician, paediatrician, or the Emergency Department determine that your child should be assessed by a paediatrician at Markham Stouffville Hospital, they may be referred to the Paediatric Assessment Clinic (PAC). The PAC can provide consultation services for your child from birth to their 18th birthday. It is staffed by a paediatrician or paediatrician/neonatologist and a registered nurse.
Reasons your child might need a Paediatric Assessment Clinic appointment
Your child might need to be seen by PAC if they have any of the following:
- Febrile illness
- Seizures
- Unwell for more than four days
- Non-emergency abdominal pain
- Infections (cellulitis, urinary tract infection, pneumonia)
- Gastroenteritis with mild-to-moderate dehydration
- Newborn feeding issues/dehydration
- Borderline weight loss (10%)
- Newborn jaundice
To be seen by a paediatrician in this clinic, you must have a Paediatric Assessment Clinic physician referral. If your child is seen in PAC, we anticipate that they will go home and not require admission to hospital.
If your child is having an emergency, proceed to the nearest Emergency Department.
Making your child’s appointment
Your child’s physician can download the Paediatric Assessment Clinic referral form here. They can fill it out and fax it to the PAC at 905-472-7535. The PAC secretary will call you with your appointment time. If you have not heard from PAC within 72 hours, please call 905-472-7534. To cancel or rebook your appointment at PAC, please call 905-472-7534 during regular business hours (9 a.m. – 4 p.m., Monday – Friday).
What to bring to the appointment
Please bring your child’s OHIP card, any discharge paperwork from the Emergency Department or previous hospital admissions if applicable, and any other relevant reports, X-rays, or films. Also bring personal items such as diapers, wipes, formula/breast milk, and a snack/lunch if needed.
