Oak Valley Health offers adult diabetes services at three different clinic locations. Adults who are 18 years old and older who are at risk of developing diabetes, who have prediabetes, type 1 or 2 diabetes, or gestational diabetes are welcome to access our services.

Our diabetes care team includes endocrinologists, registered nurse certified diabetes educators (CDE), and registered dietitian CDEs. Our care team can help you and your family learn about how to manage and live well with diabetes.

Weight management program

Oak Valley Health’s Adult Diabetes Clinic, in partnership with Carefirst Family Health Team, offers a virtual weight management program. The Adult Diabetes Clinic (DEC) is currently accepting physician referrals.

  • understand how mental health plays a role in weight management
  • have exercise resources and build self-efficacy for exercise/physical activity
  • build knowledge on healthy eating to promote mindful and SMART eating choices
  • reduce risk of weight-related comorbidities (including chronic diseases)
  • achieve 5-10% of body weight loss
  • feel confident on managing overweight/obesity condition

Group information sessions about diabetes

Group information sessions are offered on topics of interest including the following:

  • Healthy eating, active living and exercise, and weight loss management
  • Blood sugar monitoring, risk reduction, and diabetes medication management

Please note: Patients are to attend group sessions first to receive education, such as lifestyle changes, that can be implemented right away. On a case by case basis, individual appointments may be subsequent.

Referral and appointment information

A physician referral is necessary to be seen by an endocrinologist. All other appointment types can be accessed by self-referral.

It is important that you make every attempt to keep your appointment with us, especially if you have been diagnosed with gestational diabetes. However, we understand there is sometimes a need to reschedule. If you need to move your appointment, please note we are currently experiencing high volumes and unfortunately, at this time, we are not able to prioritize re-bookings, but will see you as soon as we can. Please note, this also applies for patients who miss their appointments.

Gestational diabetes

Gestational diabetes is a type of diabetes that can occur within the second or third trimester of pregnancy. Changes in diet, activity, and sometimes the addition of medication will reduce risks to mom or baby. Pregnancy is a short-term event, it is important to initiate changes early.

At Oak Valley Health’s Diabetes Clinic, when a referral is received the patient will be contacted and signed up for education sessions, held virtually every Tuesday and Thursday. Booking will be made in the next available session. A virtual appointment will be booked with a doctor, who will oversee your diabetes care for the duration of your pregnancy. Appointments are scheduled as either morning or afternoon during four-hour windows. The doctor will contact you within your scheduled four-hour window.

Best efforts will be made to book both of these virtual meetings within 7-10 days of your referral. It is important for both the patient and baby’s health to make time for both of these important appointments.

Tuesdays: 8 a.m. – 9 p.m.
Wednesdays: 8 a.m. – 8 p.m.
Thursdays: 8 a.m. – 9 p.m.
Fridays: 8 a.m. – 4 p.m.

Aaniin Community Centre Clinics, 5664 14th Ave., first floor, Markham, ON

This clinic is run Tuesday to Thursdays from 9 a.m. to 5 p.m. and Fridays from 8 a.m. to 4 p.m. By appointment only.
