Advance care planning is the process of thinking about and sharing your wishes for future health and personal care. It can help you tell others what would be important if you were ill and unable to communicate.
Assistance with advance care planning is available through your physician or the hospital’s ethicist.
Useful resources are included below. For more information, please contact our ethicist at ethics@oakvalleyhealth.ca.
