ViMos is an innovative, patient-centered self-management program that provides remote monitoring and clinical support for patients with chronic conditions, such as chronic obstructive pulmonary disease (COPD) and congestive heart failure. The goal is to reduce hospital readmissions and empower patients by providing them with the tools and support to self-manage their health from the comfort of their own home.

What can patients expect?

ViMos is designed to encourage patients to play an active role in their own health. Patients in the ViMos program receive an android tablet, oximeter, blood pressure cuff, and weight scale in order to take their vital sign readings daily. ViMos alerts patients with abnormal results and early warning signs. A ViMos clinician works with patients to review readings and symptoms in order to provide recommendations and support before their health worsens.

The results are automatically stored and can be shared with the care team. ViMos is not meant to replace emergency care. Patients should visit their nearest Emergency Department for any urgent issues.

How can patients participate?

ViMos is available to patients who have recently been discharged, those who are part of Oak Valley Health’s Markham Stouffville Hospital’s Centre for Respiratory Health and COPD clinic, General Internal Medicine Clinic, and Heart Function Clinic. Patients eligible for ViMos will be referred by a member of their care team.
