Oak Valley Health’s Transitional Care Units (TCU) are located at Sunrise of Richmond Hill and Sunrise of Unionville and are operated in partnership with Bayshore Integrated Care Solutions. Between the two Sunrise facilities, the TCU has 28 beds available.
About the Transitional Care Unit
The TCU provides care for patients who no longer require acute services of a hospital but are also not ready to go home. If you are staying at our hospitals in acute care and you no longer require this level of care, but you still need care services, you may be sent to our TCU. The TCU provides the highest quality care and an extraordinary patient experience to patients in a setting more suitable for their care needs.
Patients that are transferred to a TCU will experience a seamless transition from the hospital to their new unit.
